IMMPACT-MM: Insights into Multiple Myeloma Patient Outcomes Following Ciltacabtagene Autoleucel Treatment in Earlier Lines of Therapy in the Real World

Patients with relapsed or refractory multiple myeloma (RRMM) – a blood cancer that has returned or stopped responding to treatment – often require multiple rounds of therapy as their disease progresses. Clinical trials have shown that ciltacabtagene autoleucel (cilta-cel), a CAR-T cell therapy, can help patients with RRMM remain free from disease progression without requiring subsequent treatment. Following expanded regulatory approval, cilta-cel is now available earlier in the treatment journey for those patients. As its use expands, real-world evidence (RWE) is needed to determine whether the outcomes observed in clinical trials are replicated in routine clinical practice.
To understand how well trial outcomes translate to patient care, Analysis Group Vice President Bruno Emond and Manager Jessica Maitland collaborated with investigators from Memorial Sloan Kettering Cancer Center, Johnson & Johnson, Legend Biotech USA, and UT Southwestern Medical Center on a study of patients with RRMM treated with cilta-cel after one to three prior lines of therapy. The study used medical records and physician notes from the largest US real-world cohort to date evaluating minimal residual disease (MRD) negativity (i.e., that no cancer cells were detectable after treatment) among patients with RRMM who received cilta-cel as an early-line therapy. The researchers assessed how those patients responded to treatment, whether any cancer remained detectable after treatment, and whether their disease worsened or required additional therapy during follow-up.
In an article on their findings, the researchers report that – among patients evaluated for MRD – 97.2% had no detectable remaining cancer after treatment with cilta-cel. Together with a high overall response rate (98.0%) and observed proportion of patients who remained free from disease progression and did not require further therapy during follow-up (95.8%), the study's findings provide early RWE of rapid and effective disease control among patients with RRMM who were treated with early-line cilta-cel, reflecting outcomes previously observed in clinical trials. Longer follow-up and repeated MRD measurements may provide further insights into MRD as a prognostic marker for response durability and survival outcomes, helping to contextualize the potential role of cilta-cel in achieving a functional cure for patients with RRMM.
